In-Home Care Options for Disabled Adults
We offer two primary options for in-home disability care: visiting care and live-in care. Live-in care involves a disability support worker living with the client to provide assistance whenever necessary, 24 hours a day. This approach allows for flexibility in meeting the client’s specific needs, and our compassionate and adaptable caregivers will assess the client’s daily routine to establish a relationship that fosters optimal well-being and peace of mind. Visiting care may be more suitable for disabled adults who want to maintain their independence and do not wish to have a caregiver living with them. With visiting care, the client can continue to reside at home, and a caregiver will visit them daily.
Comprehensive Care Plan
Livewell has dedicated Case Managers who work closely with the participant and their loved ones to create a detailed Care Plan that aligns with the participant’s goals, desires, and requirements. After consulting with a healthcare expert, we create a personalized 24-hour Care Plan and home care package that covers all the areas of support we offer, and outlines how we can assist in maintaining your way of life.
